Thank you for your interest in Young Minds Development Academy. Enclosed are the forms you will need to complete and bring with you on or before your child's first day.

Step 1
If you haven’t done so already, please go to our website: www.ymdaseattle.com, click “ENROLL NOW” and complete your pre-registration. This form is not accessible via a mobile device. You must a Computer, tablet only (or you may complete the full registration paper form by request)

Step 2
Complete the highlighted forms (if you don’t have a way to print and complete, you can call us and arrange to pick these forms up from the center):

1. Parent Handbook - please read for understanding our policies
2. Parent orientation checklist requires your initials and signature for your understanding of the handbook
3. The EIEA is required for our food program (very important to be completed entirely, and is date sensitive, the date cannot appear to be changed/scratched out or it would require a do-over, if you receive food stamps, your case number is required)
4. COVID screening upon arrival is for information only, questions you must answer to drop off your child each day
5. Certificate of Immunization (1 per child) required before the 1st day
6. Current year Closures - FYI
7.Emergency Consent - Complete all areas of this form

Step 3
Drop completed forms off to Young Minds and schedule your over the phone orientation. Please note: Orientation must be completed at least 2 days before your child's anticipated start date.

INFANT (6wks-12 mo) & TODDLERS (12mo-36 mo) ONLY: These classes are funded by an Early Head Start Grant and requires families to complete a separate registration packet in addition to the enclosed. Once you complete all instructions in this email, you will receive a phone call within 24 hours from our Family Advocate that completes the enrollment process for this program with an explanation of the program requirements. This process will take some time, but don’t worry, you WILL NOT need to complete this portion before your child’s first day.
